MY VOICE, MY CHOICE - OHIO'S PROVIDER REPORT CARD RESULTS ARE NOW AVAILABE ON www.thearclink.org. CONSUMERS, FAMILY MEMBERS, PROVIDERS AND COUNTY BOARDS CAN SEARCH FOR REPORT CARD RESULTS BY COUNTY!

A statewide pilot of the My Voice, My Choice project began in April 2003. Individuals who receive residential services in Ohio are being contacted on a county-by-county basis to participate in the project. Report card grades are being made available as each county is completed. Check the Report Card Results page on this web site for results. Each county will also be provided with a written summary of the results for distribution to consumers and family members.

The first pilot project was completed in December 2004. A second round of interviews is now underway. Report card grades for 2005 are being made available as each county is completed.

NEW IN 2005!!! We are introducing a new simplified grading scale. We will also be reporting results compared to the 2004 statewide average for each question on the report card. This new reporting process will give consumers, families and providers more information about how their services compare to other providers in the state. Due to the new grading scale, results for 2004 have been removed from the web site.

The process for interviewing is:

	A phone call is made to the County Board of MRDD to set up an Orientation Meeting.
	Project Directors hold the Orientation Meeting with county board staff to explain the project and request assistance with accessing eligible individuals. The process for face-to-face interviews is explained. The process for mailing guardian packets is explained.
	Date(s) are selected for face-to-face interviews.
	Independent Contract Interviewers are assigned to conduct interviews.
	Interviews are conducted with all eligible individuals. (Face-to-face interviews will only be conducted with individuals who are their own guardian. Report cards will be mailed to guardians).
	Interviewers submit data to Project Directors
	Project Directors compile data, determine report card grade for each provider and publicize results via www.thearclink.org and a written booklet designed for each county.
	Report Card results are mailed to all providers at least 10 days prior to publication.
